You see the patient.
We finish the
documentation.
An AI scribe, dictation, or handwritten at the bedside — however the note starts, a physician still has to stand behind it. We check it against the record and enter it into your EMR. You read it and sign.
What Your Practice
Could Reclaim
An AI scribe still leaves the checking, the corrections and the EMR entry with you — and that time comes out of clinic.
Put in your own figures and see what it is worth to hand that work to a licensed physician who reviews every note against the record before it reaches you to sign.
- Works with your existing AI scribe, dictation or handwritten notes
- Reviewed by a physician, not an automated quality check
- Entered into your EMR, so the chart is finished when you open it
Checking each note and transferring the information accurately into the EMR, after the visit itself.
How this is modelled. Additional visits are counted only when they fit inside the workday length you selected, alongside your existing consultations and a fixed two-minute sign-off per note; pre-existing spare time is not credited to SwiftWorks. Breaks, administrative tasks and other workflow constraints are not modelled, and patient demand is not guaranteed. The two-minute sign-off will vary by patient and does not remove physician responsibility for accuracy and signature. Revenue shown is gross collected revenue, not profit, and excludes SwiftWorks fees. The value of reclaimed time and the additional-visit revenue measure the same benefit two ways; they cannot be added together.
A Different Standard
of Documentation
SwiftWorks Solutions LLC is a U.S.-registered company providing physician-led clinical documentation support to practices, clinics and healthcare institutions across the United States.
Some practices now run an AI scribe. Others dictate, and some still write by hand. SwiftWorks works with all of them, because the part that matters is not how a note gets drafted — it is that a clinician has to stand behind it. Whatever produced the first version, a physician reads it against the record and takes it the rest of the way into the chart.
Leadership
Dr. Abdul Rehman Pasha
CEO & Founder“As a physician, I saw firsthand how much valuable time is lost to documentation—and how easily rushed or incomplete notes can create problems later. That’s why I created SwiftWorks: to build the kind of documentation support I would want beside me in my own practice. Physician-led, clinically informed, and focused on getting the record right the first time. The goal is simple: give healthcare professionals more time for their patients, with confidence that their documentation is being handled by people who truly understand it.”
The Work That Lands
After Clinic
The Review Still Falls To You
An AI draft or a typed-up dictation still cannot sign itself. Every note needs a clinician's eyes before it becomes part of the record — and that time comes out of the same day.
A Draft Is Not A Chart
Getting the documentation into the EMR — correct fields, correct encounter, correct format — is still manual work that lands after clinic rather than during it.
Nothing Checks It Against The Record
Software hears one encounter; a general typist hears words. Neither cross-checks the medication list, questions something that contradicts the history, or notices what was never said aloud. A 2026 review in BMJ Digital Health & AI concluded that every AI-generated note still needs a clinician to read it.
Errors Surface Late
A problem found at the end of the day, or after a claim comes back, costs far more to fix than it would have cost to catch before the note was filed.
We Work With How You Already Work
Run an AI scribe, dictate into a recorder, or write on paper — we fit around whichever it is. We are not here to change your workflow. We are here to make sure what comes out of it is something you can put your name to.
Send It However It Comes
An AI draft, an audio file, handwritten notes, or a chart already sitting in the EMR. Send it over and move on to the next patient.
Read By A Physician
A practising clinician reads every note after the encounter — terminology, structure, medications, and anything that does not sit right against the rest of the record.
Into The EMR, Correctly
Notes, diagnoses, investigations and referrals entered where they belong, at your direction — or what is already there reviewed and corrected.
It Doesn't Come Back
The point of the service is that you do not reopen the note. No end-of-day correction queue, no rework after a query lands.
Core Services
Clinical Documentation Review
Every note read against the record after the encounter — catching what was mis-stated, left out, or does not fit the rest of the chart.
- Review of dictated and AI-drafted notes alike
- Verification against the patient record
- Medication, allergy and history consistency checks
- Terminology, structure and omission corrections
- Clinical discrepancies flagged for your confirmation
- Works alongside any ambient or dictation-based tool
EMR Documentation & Entry
Your documentation entered and organised in the right places, under authorised access and at your direction.
- Consultation notes entered in the correct chart fields
- Diagnoses, investigations and referrals recorded as you direct
- Medication entries prepared and left for your sign-off
- Charts already in the system reviewed and corrected
- Practice Fusion and other major systems supported
Medical Transcription
Dictation and recorded encounters documented by someone who understands the clinical language, not a general typing pool.
- SOAP notes with strict clinical formatting
- Patient encounter documentation
- Operative and procedural notes
- Discharge summaries and consultation reports
- Emergency department documentation
- Handwritten and legacy records made digital
We Flag What Doesn't Fit.
You Decide What's Right.
Finding a discrepancy is only half the job. What a service does next is the part that decides whether you can trust the record.
A clinically significant detail is never altered on our judgement alone. We surface it, show you the context we based it on, and leave the decision with you — so the chart reflects what you decided, not what someone assumed.
“… patient tolerated the endoscopy well, discharged in stable condition.”
Indication, bowel preparation and findings all describe a colonoscopy.
Flag it for your confirmation, with the conflicting detail quoted. The note is not changed until you say so.
See the Output
Whether it started as dictation, handwriting or an AI draft, this is what comes back — and what goes into the chart.
Illustrative excerpt only. This is a fictional composite patient, shortened for display, written to demonstrate structure and formatting. Delivered notes carry the full detail described below. No real patient information is used, shown, or derived from any actual encounter.
Chief Complaint: Annual preventive physical examination. No acute concerns reported.
What you receive: The chief complaint alongside a complete review of systems — every relevant system documented, not just the presenting problem.
General: Well-nourished, well-developed, in no acute distress.
Heart: Regular rate and rhythm, no murmurs.
Lungs: Clear to auscultation bilaterally.
What you receive: A detailed physical examination with findings written out system by system, plus any laboratory or radiological results incorporated directly into the note rather than left as separate attachments.
Routine health examination — (Z00.00)
Bilateral cerumen impaction — (H61.23)
What you receive: Every assessment arrives with its ICD-10 code already attached. Your team stops losing time looking codes up, and less of the coding burden lands downstream.
Preventive screening completed. Laboratory studies ordered per age-appropriate panel. Follow-up as scheduled.
What you receive: The complete plan documented to payer standards, so the encounter is properly supported when the claim goes out and denials traced back to insufficient documentation become far less likely.

What Sets Us Apart
Physician-Led Documentation
Software can draft a note. It cannot be accountable for one. Every document that passes through SwiftWorks is read by a clinician, and as we grow, additional physicians and qualified documentation professionals are onboarded under the same standard of review.
- Clinically informed review of every note
- Context-aware review, not keyword matching
- Reduced documentation errors
- Documentation aligned with real clinical decision-making
Turnaround & Reliability
Delivery built around the operational rhythms of a working clinical practice.
- Fast and consistent turnaround times
- A dedicated point of contact for every client
- Scalable support based on your workload
- Continuity of the same documentation team
HIPAA & Information Security
Clinical documentation work involves access to protected health information. SwiftWorks treats that as the first requirement of the engagement, not an afterthought — and is prepared to document its security practices in detail during onboarding.
A Business Associate Agreement is available and can be executed before any protected health information is exchanged.
Safeguards
- Business Associate Agreement (BAA) available
- Role-based access to patient information
- Minimum-necessary PHI access principles
